How to use it without creating another stale spreadsheet
Assign one owner to each row, use the next-action date as the working control, and attach a tracking ID or evidence reference whenever the status changes. Replace the sample row before operational use. Keep source documents in a separate controlled folder; the CSV should point to evidence rather than become the only evidence.
For Medicare revalidation dates, use the current CMS lookup rather than estimating a due date from memory. For CAQH, keep the credentialing attestation cadence separate from practice-location confirmation. For commercial payers, record the payer/product/location scope because a generic “approved” status can hide an incomplete contract or billing activation.
