Every guide should answer one operational question. We prefer a narrow page that identifies the right record, decision, workflow, and source over a long article that repeats the same advice under different headings.

Source hierarchy

CMS and CAQH first-party material is preferred for rules those organizations control. Commercial payer claims are kept general unless supported by that payer’s own published materials. We do not invent processing times, effective dates, payer fees, or state requirements.

Corrections

When a source changes or an error is identified, the article should be corrected at the factual level and the related workflow reviewed for downstream effects. Reviewed dates are updated only when the content is actually checked.

Editorial workflow

Each guide is reviewed for source accuracy, scope, internal consistency, and practical usefulness before publication. Reviewed dates change only after the underlying guidance and cited sources have actually been checked. The repository intentionally blocks indexing when no responsible editor name is configured.

No fake authority

The site does not claim affiliation with CMS, CAQH, Medicare Administrative Contractors, or commercial health plans. We do not fabricate certifications, government seals, payer endorsements, or expert biographies.
